Encompass Links2Health is a comprehensive program designed to provide wraparound care to community members to help them meet essential life needs and improve health outcomes. Made possible by new Medi-Cal funding, our program offers an exciting new way to provide comprehensive case management and care coordination for Medi-Cal beneficiaries. We work to reduce barriers to quality care and break down the traditional walls of health care to meet those we serve wherever they are - in the community, in a shelter, in their doctors' office or at home - to help them meet their greatest health needs.
Additionally, through Links2Health, we can connect individuals to Community Supports to meet their social needs, including housing navigation, housing retention and sustaining services.
If you or someone you know could benefit from support connecting to housing, linking up with a primary care home, accessing transportation to appointments or a host of other social services, Links2Health may be able to help! To be eligible for Links2Health, you must be a Medi-Cal member and one of the following:
- Adults or families experiencing homelessness
- Adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)
- Adult high utilizers of the healthcare system

Community partners can also refer Medi-Cal beneficiaries through the Unite Us platform. Be sure to include the following in the notes section:
- The client's Central California Alliance for Health number (if available)
- A brief note about why they need services
