Thank you for your interest in our child development programs. Our high quality child development programs for infants, toddlers and preschool age children are available for families living in Santa Cruz County. Our Early Head Start, Head Start and State Preschool programs are no cost for income eligible families.

CHILDREN WITH DISABILITIES

Your child may be eligible for one of our programs if they have a current Individualized Education Plan (IEP) or Individual Family Service Plan (IFSP). With a current IEP/IFSP income eligibility may be waived.

REQUIRED DOCUMENTS

Include copies of the following documents with your child’s application (documents will not be returned):

• Copy of child’s immunization record OR Birth Certificate OR Hospital Certificate OR Medical Card Income Verification
• Income Verification documentation -- must include all sources of income received by the child’s parents or guardians (see Income Eligibility section of application for more detailed information) OR documentation of CalWORKS/SSI
• Copy of IEP or IFSP (if child has a diagnosed disability)

SUBMITTING YOUR APPLICATION

Remember, to sign and fill out application completely. Incomplete applications cannot be processed. You can submit your child’s application in person or mail it to the address above. You will be contacted by mail once your child’s application has been processed. All information is kept confidential. It will be used to help us determine if your family is eligible for our programs and to prioritize (using our selection criteria) your child’s placement on the Wait List.
